Your appointment will not be confirmed until you complete and send the required paperwork through the website, including a patient Health Declarations Form for Covid-19.
Masks must be worn at all times for entry into the office building. Once in the office, you may remove your mask only for procedures involving the mid- or lower face. Dr. Madhere will not remove her mask and will wear a facial visor.
Your temperature will be taken upon entry into the office. Dr. Madhere reserves the right to refuse your treatment if your temperature is elevated.
In an attempt to create a seamless paperless environment, when scheduling an appointment, your credit card information will be required. It will not be charged unless you cancel outside of the cancellation period or have an unresolved balance.
